Student Requirements
Requirements Upon Admission to Butler University
DUE AUGUST 1ST – If Health Services does not receive the following information, if it is incomplete, or required immunizations are non-compliant, a hold will be placed on a student’s account after August 1, 2024 and they will not be able to complete registration for the spring semester until requirements are met.
1. Health Forms and Questionnaires
Students are required to submit the following documentation in English prior to beginning classes by August 1 for fall admission or January 1 for spring admission.
- Physical exam:
- The required physical is to be performed and signed by a medical provider and must have occurred within the 12 months prior to submission deadline (8/1/23 or after).
- This can be on our Physical Examination Form or your provider’s form.
- This can be submitted to healthservices@butler.edu , uploaded through the health portal, or faxed to 317-940-6403.
- You may have a physical completed at Butler Health Services once you are on campus if needed. Please call 317-940-9385 to schedule.
- Immunization Records:
- The vaccines required by Butler can be found here.
- The immunization records must be on official medical office letterhead or state immunization registry stationary.
- Records that are pulled from within an individual’s private health portal will be accepted only if their name and date of birth are clearly visible.
- These can be submitted to healthservices@butler.edu , uploaded through the health portal, or faxed to 317-940-6403.
- Online forms and questionnaires on My Health Web Portal:
- Once you have your Butler ID, sign in to the My Health Web Portal (myhealth.butler.edu)
- Click on “Forms” on the left side menu and complete the:
- Health History Form
- Tuberculosis Screening (a second one is prompted by any “yes” answers)
- TeleMedicine Consent
Records are reviewed for immunization compliance, medications, allergies, chronic diseases, or medical conditions that might require special attention while you are a student at Butler University.
If you have a medical condition that has special requirements or accommodation needs please contact Student Disability Services for additional support.
Our records are only shared with your written permission.
Health Services takes the following steps prior to placing a hold on student registration:
- Audits all records for complete compliance
- Sends personalized messages to students about deficient information
- Sends a series of broadcast emails to all students through Butler Today daily e-announcements
2. Proof of Health Insurance is an Annual Requirement
It is a policy of the University to require that students possess health insurance while attending Butler University. On an annual basis, students can “Waive Coverage” or “Opt-In” to the University-sponsored United Healthcare Plan by following the link sent directly to their Butler email from healthinsurance@butler.edu.

Undergraduate and graduate status is defined below:
- Full-time, degree-seeking undergraduates
- Full-time graduate students with 6 credits or more
- All international students (in both undergrad and graduate programs) will be automatically enrolled in the University-sponsored plan (with the exception of ISEP participants)
- These students do NOT need to complete the waiver – Health Services will process this for them
Students who take no action by the enrollment deadline will be automatically enrolled in and billed for the University-sponsored plan.
Due to FERPA/HIPAA regulations, all communication regarding the student’s health information must be with the student.
Medical and Religious Exemptions
Exemption requests can be submitted for approval by our team. Medical requests must be completed and signed by a medical provider and will be reviewed by Dr. Counsell, while religious requests will be reviewed by our director.
