Program Licensure Disclosures

Butler University has several programs that will prepare students for licensure or professional certification. These programs are:

Butler’s curriculum will prepare you to meet the professional licensure or certification requirements of Indiana. 

If you are completing your program on our campus, you should be aware of licensure requirements and how they can vary by state, but you are not required to complete any additional paperwork or process in order to enroll and complete the program. If you have plans to work in a state where Butler’s program does not meet licensure or certification requirements, you should review the requirements for your state closely and consider how you will meet the additional requirements prior to licensure. 

If you are completing your program of study online, Butler is only permitted to enroll you if:

  1. You live in a state where Butler’s program meets the licensure requirements for employment OR
  2. You voluntarily complete an attestation form saying you’ve been made aware of the requirements, that Butler’s program does not meet those requirements, and you still wish to enroll in the program. 

There are several reasons why you might wish to complete the voluntary attestation form. These include:

  • You live in a state that does not meet the requirements, but you are licensed by and work in a state where the program does meet the requirements.
  • You live in a state where the program does not meet the requirements, but you plan to move to and live in a state where the program does meet the requirements.
  • You prefer Butler’s program and have carefully reviewed the additional requirements that will need to be completed to meet requirements in a state, and wish to complete those additional requirements in addition to Butler’s program. 

Instructions for Completing the Voluntary Attestation Form

If you are an online student who lives outside of Indiana, follow these steps to complete the Voluntary Attestation Form

Step 1: Carefully review and consider state licensing requirements in the context of your personal and professional goals and plans. 

Carefully review the program licensure information on this web page and the specific program information for your program. Butler University is required to make this disclosure to you at the time you enroll in the program. We are not able to provide ongoing updates to enrolled students about changes in curriculum and the impact they have on your personal and professional goals. As a professional in your chosen field of study you must continue to stay abreast of information by keeping up to date on state licensing requirements and guidelines for all states that are relevant to your individual plans and goals.

Step 2: Ask for help if needed.

Talk with Butler University admissions or reach out to the state licensing authority to help you understand the licensure requirements for a state. 

Step 3: Complete the Student Attestation form

You will need to complete the voluntary attestation form if you meet the following three conditions:

  1. You are enrolling in an online program that leads to licensure or certification AND
  2. You live in a state where Butler’s program does NOT meet the licensure or certification requirements AND
  3. You wish to proceed with enrollment in Butler’s program as your plans include working in a state where the program does meet requirements OR you plan to meet additional requirements in some other way.

If these three things are all true, complete the Student Attestation: Professional Licensure Programs Form to confirm your understanding of the program’s licensure limitations and outline your plan for obtaining licensure or certification after graduation.
