If Health Services does not receive the following information, if it is incomplete, or required immunizations are non-compliant, a hold will be placed on a student\u2019s account after August 1, 2026, and they will not be able to complete registration for the spring semester until requirements are met.<\/strong><\/p>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-1-health-forms-and-questionnaires\">1. Health Forms and Questionnaires<\/h4>\n\n\n\n<p>Students are required to submit the following documentation in English prior to beginning classes by <strong>August 1\u00a0for fall admission<\/strong>\u00a0or\u00a0<strong>January 1\u00a0for spring admission<\/strong>.<\/p>\n\n\n\n<h6 class=\"wp-block-heading\" id=\"h-physical-exam\">Physical Exam<\/h6>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The required physical is to be performed and signed by a medical provider and must have occurred within the 12 months prior to submission deadline (<strong>8\/1\/25<\/strong> <strong>or after<\/strong>).<\/li>\n\n\n\n<li>This can be on our <a href=\"https:\/\/cdn.butler.edu\/www\/sites\/23\/2025\/07\/24111120\/Physical-Form-2025-26.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Physical Examination Form<\/a> or your provider&#8217;s form.<\/li>\n\n\n\n<li>This can be submitted to <a href=\"mailto:healthservices@butler.edu\">healthservices@butler.edu,<\/a> uploaded through the health portal, or faxed&nbsp;to 317-940-6403.<\/li>\n<\/ul>\n\n\n\n<h6 class=\"wp-block-heading\" id=\"h-immunization-records\">Immunization Records<\/h6>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The vaccines required by Butler can be found <a href=\"https:\/\/cdn.butler.edu\/www\/sites\/23\/2025\/10\/20100108\/Immunization-Req-2025-2026.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>.<\/li>\n\n\n\n<li>Immunization records must be on official medical office letterhead or state immunization registry stationary.<\/li>\n\n\n\n<li>Records that are pulled from within an individual&#8217;s private health portal will be accepted <strong>only <\/strong>if their name and date of birth are clearly visible.<\/li>\n\n\n\n<li>These can be submitted to <a href=\"mailto:healthservices@butler.edu\">healthservices@butler.edu<\/a>, uploaded through the health portal, or faxed to 317-940-6403.<\/li>\n<\/ul>\n\n\n\n<h6 class=\"wp-block-heading\" id=\"h-online-forms-on-my-health-web-portal\">Online Forms on My Health Web Portal<\/h6>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Once you have your Butler ID, sign in to the&nbsp;<a href=\"https:\/\/myhealth.butler.edu\/\" target=\"_blank\" rel=\"noreferrer noopener\">My Health Web Portal<\/a>&nbsp;(myhealth.butler.edu).<\/li>\n\n\n\n<li>Click on \u201cForms\u201d in the side menu and complete the:\n<ul class=\"wp-block-list\">\n<li>Health History<\/li>\n\n\n\n<li>Tuberculosis Screening (a second one is prompted by any &#8220;yes&#8221; answers)<\/li>\n\n\n\n<li>TeleMedicine Consent<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>If &#8220;Forms Download&#8221; is giving you issues, do not worry; we will make this compliant on the back end once the above three items are complete.<\/li>\n<\/ul>\n\n\n\n<p><em>Records are reviewed for immunization compliance, medications, allergies, chronic diseases, or medical conditions that might require special attention while you are a student at Butler University.<\/em><\/p>\n\n\n\n<p><em>If you have a medical condition that has special requirements or accommodation needs please contact&nbsp;<a href=\"https:\/\/www.butler.edu\/academic-services\/disability\/\">Student Disability Services<\/a>&nbsp;for additional support.&nbsp;<\/em><\/p>\n\n\n\n<p><em>Our records are only shared with your written permission.<\/em><\/p>\n\n\n\n<p>Health Services takes the following steps prior to placing a hold on student registration:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Audits all records for complete compliance<\/li>\n\n\n\n<li>Sends personalized messages to students about deficient information<\/li>\n\n\n\n<li>Sends a series of broadcast emails to all students through Butler Today daily e-announcements<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\" id=\"h-2-proof-of-health-insurance-is-an-annual-requirement\">2.
