Certificate of Insurance

On-campus events with equipment or food (in addition we need proof of food license) or higher risk events off campus, student organizations must request a "Certificate of Insurance." This should accompany a contract.

Insurance Requirements

Please request for Butler University to be listed as additionally insured.

The policy must provide the following coverage and limits as a minimum:

  • $2,000,000 per occurrence under general liability

                      Or

  • $1,000,000 per occurrence under general liability and $1,000,000 per occurrence under umbrella liability

Sample Butler University Insurance Requirements (Commercial Users)

It is agreed that the Lessee shall purchase and maintain at its expense liability insurance protecting itself and Butler against loss or expense by Lessee's use of the leased premises.  A certificate of insurance designating Butler University as an additional insured under General Liability shall be submitted at least 30 days prior to the event. If Butler has not received Lessee's proof of insurance by this date, Butler shall have the right to terminate this Agreement upon written notice to Lessee and retain the deposit.  Insurance as set forth below shall be written by a company with an AM BEST'S rating of A, VII or better with an admitted carrier satisfactory to Butler University.

Said insurance shall provide limits of General Liability of not less than the following:

General Liability

  • General Aggregate $1,000,000
  • Products & Completed Operations Aggregate $1,000,000
  • Personal and Advertising Injury $1,000,000
  • Each Occurrence $1,000,000
  • Fire Damage (any one fire) $50,000
  • Medical Expense $5,000

Automobile Liability

  • Liability (Combined Single Limit) $1,000,000
    (Coverage to apply to all owned, hired and non-owned vehicles)

Workers Compensation

  • Part I—Workers Compensation Statutory Benefits
  • Part II—Employers Liability $500,000 each accident
  • $500,000 each employee
  • $500,000 policy limit

Umbrella

  • Limit $1,000,000 Per Occurrence
  • Retention $10,000

Butler shall not be liable on any contract of the Lessee, or on account of any act of negligence on the part of the Lessee, or any damage caused by the Lessee, or any employee and/or worker as designated by the Lessee thereof and that the Lessee will pay for any and all damages to the premises, building and its equipment, other than that which is considered to be normal wear and tear. Butler shall also not be liable for any illness or injury incurred by the lessee, its guests, agents, employees, or independent contractors.

The "vendor" must carry General Liability insurance with a per occurrence limit of not less than $2,000,000 combined single limit for bodily injury and property damage. The limit of $2,000,000 can be achieved by the General Liability policy alone, or with a combination of General Liability and Umbrella coverage. If the vendor will be serving or supplying alcohol, the insurance coverage must also include Liquor Liability with a limit of no less than $1,000,000 per claim. The vendor shall also provide proof of Workers Compensation coverage which includes Employer's Liability of not less than $100,000, each accident; $100,000, each employee by disease; and $500,000, per policy by disease. Proof of all coverages listed above shall be in the form of a certificate of insurance showing Butler University (and other entities to be determined) as an Additional Insured under General Liability. The certificate shall be submitted no less than seven (7) days prior to the event, with carriers satisfactory to Butler University, and with an AM Best rating of A, VII or better.
