Student Requirements

New Students

Requirements Upon Admission to Butler University

1. Health Forms and Questionnaires

We require all domestic undergraduate and international students to submit the following documentation in English prior to beginning classes by August 1 for fall admission or January 1 for spring admission.

  1. Physical exam:
    1. The required physical is to be performed and signed by a Medical Provider. 
    2. The physical must have occurred within the last 12 months.
    3. This can be submitted on the Physical Examination Record form or your doctor’s form.
    4. You may have a physical completed at Butler Health Services once you are on campus if needed. Please call to schedule 317-940-9385.
  2. Immunization Records:
    1. The immunization records must be on official medical office letterhead or state immunization registry stationary.
    2. See the back of the Physical Examination form for details.
    3. This can be submitted to healthservices@butler.edu or  Fax  to 317-940-6403.
  3. Health History Form online on My Health Web Portal
    1. Once you have your Butler ID, sign on to  the My Health Web Portal (myhealth.butler.edu)
    2. Click on "Forms" on the left side menu and complete the Health History Form
  4. Tuberculosis Screening on My Health Web Portal
    1. Once you have your Butler ID, sign on to  the My Health Web Portal (myhealth.butler.edu)
    2. Click on "Forms" on the left side menu and complete Tuberculosis Screening

Records are reviewed for immunization compliance, medications, allergies, chronic diseases, or medical conditions that might require special attention while you are a student at Butler University.

If you have a medical condition that has special requirements or accommodation needs please contact Student Disability Services for additional support.

2.Proof of Health Insurance is an Annual Requirement

It is a policy of the University to require that students possess health insurance while attending Butler University.  

  • On an annual basis, students can "Waive Coverage"  or "Opt-In" to the University-sponsored United Healthcare Plan by visitng this webpage and follow the instructions.
  • Visit Student Health Insurance for more information.
  • Undergraduate and graduate status is defined below.
    • Full-time, degree-seeking undergraduates
    • Graduate students with 6 credits or more
    • International undergraduates must enroll in the University-sponsored plan (with the exception of ISEP participants)
    • International graduate students are eligible to enroll in the University sponsored plan.

 Students who take no action by the August billing date will be automatically enrolled in and billed for the University-sponsored plan. 

Due to FERPA/HIPAA regulations, all communication regarding the student’s health information must be with the student.

Incomplete Requirements

 If Health Services does not receive the information, if the information is incomplete or immunizations are required, a hold will be placed on a student’s account on August 1 and you will not be able to complete registration for the following semester until requirements are met.

Returning Students
