Schedule an Initial Appointment
In order to complete your online appointment scheduling, you will first need to complete our intake forms. Please note that this information will not be reviewed by CCS staff until the time of your appointment., Therefore, if you have an urgent need or need immediate help call our office at 317-940-9385 during business hours, call BUPD via 911, or call a local or national 24-hour resource such as St. Vincent Stress Center at 317-338-4800 or the National Suicide Hotline at 800-273-8255.
If you feel your situation is time sensitive (but not an emergency) and you need an appointment sooner than those that are available online, call our office to talk over your options for care. Also, if you need to change your initial appointment that you initially signed up for, please call our office to do so.
After scheduling an appointment online, our office will email you an appointment reminder prior to your appointment. This email will also contain a link to complete a brief questionnaire regarding symptoms during the past two weeks that we request you fill out prior to your appointment. During the pandemic, please note that all appointments are being conducted virtually via Zoom.
