The Word documents are fill-in forms that can be completed in Word and printed.
All application forms, with backup documentation, should be submitted to the Butler Institute for Research and Scholarship - scanned pdfs can be sent to IRB@butler.edu, or hard copy can be dropped off in JH109 (unstapled and printed on one-side only).
