Butler University strives to maintain a safe environment for
it's faculty, staff, students, visitors, alumni, and all other
affiliates.
Non-Employee injuries (including students, visitors, alumni, or
contracted employees). If you experience an injury, the Non-Employee
Incident Investigation Report must be completed. Note: Only
Faculty/Staff may fill the form out. If your injury is
beyond first aid treatment, the Butler University Police Department
must also be notified at 317-940-9396.
