Faculty Activity Report Template
As part of Butler University's annual faculty evaluation
process, all full-time faculty members are expected to complete an
Annual Faculty Activity Report. Full-time faculty who are not
tenure-line should address only those areas relevant to their
contractual agreement. This information will be used by your
department or program chair or dean in your annual review.
Please respond briefly and succinctly to each item that applies to
your appointment. If an item does not apply to you, please
enter "not applicable."
Your responses should refer to activities that occurred in the
2011 calendar year. To assist you in completing your Report,
the Office of Institutional Research and Assessment will distribute
faculty teaching information (courses and numbers, teaching credit
hours, term taught, enrollment, and other components) in
November.
Return your Annual Faculty Activity Report as a Word attachment
via email to your Dean's office by January 18,
2012.
