Rewards Program - Spring 2008 - Coming Soon!
Thank you to everyone who participated in the Fall 2007 Rewards
Program! Winners have been contacted via email. We hope you will
participate again this Spring!
How can you earn points?
5 points for:
- Participating in 30 minutes of exercise in one day
- Eating 5 servings of fruits/vegetables in one day
- Taking the stairs instead of the elevator (when
possible)
- Drinking 64 fl. oz. of water in one day
10 points for:
- Bringing a healthy lunch to work
- Getting at least 7 hours of sleep at night
- Replacing soda, diet soda, and high-sugar beverages with
water
20 points for:
- Wearing your seatbelt every time you're in the car (driver OR
passenger)
- Taking a multivitamin or your prescribed medications for one
full week
- Obtaining at least 150 minutes of exercise in one week
- Bringing a healthy snack to work to share with
co-workers
30 points for:
- Attending any Healthy Horizons program (ex: the noon-time
walking program)
- Registering with INShape Indiana (

- Reaching your own personal health goal
- Having your health screening or completing the on-line Health
Risk Assessment
- Having an osteoporosis screening (women only)
- Having a prostate cancer detection screening (men only)

75 points
Level 2 - Golden Paw -
150 points
Level 3 - Platinum Paw -
