Health Risk Assessment
You are welcome to participate in the Healthy Horizons Program
without having a health screening by completing a Health Risk
Assessment, a questionnaire designed to assess your health
status and to inform and provide feedback to help reduce
your health risks. Your responses are confidential and are
reviewed by a clinical faculty member from the College of Pharmacy
and Health Sciences to provide you with recommendations to
improving your health and assist you in making healthy choices.
