Do I need to make an appointment to be seen at Health Services?

To best utilize your time and ours, call ahead (317) 940-9385 to
make an appointment. In most cases we can see you the same day. If
you walk-in you will be seen on a first come first served basis, be
evaluated by a nurse, and scheduled to see the doctor if
needed.
How much does it cost?

You are responsible for the co-payment listed on your insurance
card. All charges may be charged to your student account. There is
no need to have cash on hand in order to be seen.
Do you bill insurance companies?

Yes. Bring your insurance card and your student ID to the clinic
when you visit. A list of currently accepted insurance companies
can be found on the insurance billing
page.

What immunizations does Butler require?

New
students must submit an official immunization record to Health
Services by established deadlines. Requirements are based on
Indiana state law and recommendations by the CDC and the American
College Health Association.
TD (Tetanus and Diphtheria)/ Tdap
(Tetanus/diphtheria/pertussis)
- Adult dosage must be within last ten years
- Hepatitis B, series required
- A series of three shots -- must have started series prior to
arrival on campus
Varicella (Chicken Pox)
- Must have recorded date of disease or date of
immunizations (a series of two injections)
- May provide the date of the actual illness, verified by medical
professional
- May provide proof of serum titer immune results
MMR (Measles, Mumps and
Rubella)
- Dates for two doses
- Proof of confirmed disease, with verified date provided by
medical professional
- May provide proof of serum titers immune results
- Born before 1957
TB Questionnaire required (Located in
MyHealth portal)
TB Skin Test (Tuberculin)
- TB skin test may be required based upon responses to TB
Questionnaire (if required, our staff will contact you).
- If you are an international student.
- NOTE: If you have had a positive TB skin test in the past we
will need a copy of the TB test result and a copy of the chest
x-ray report if completed as a part of a TB work
up/evaluation/treatment.

What do I do if I get sick when Health Services isn't open?

Students who call after hours will get an answering machine
advising them what to do. Some options include:
- If it is not urgent, leave a message and a nurse will call them
back on the next business day.
- Call BUPD who in turn may page a nurse
- Go to a nearby urgent care center or emergency room

Will you write a note because I missed class?

Health Services staff members do not write excuses for missing
class time. It should also be noted that professors will be unable
to check and see if students were at the clinic. Due to privacy
regulations, we are unable to confirm if students received
services.
Why won't you discuss my visit to the Health Services with my parents?

We take medical confidentiality seriously. We are prevented by
law from releasing any medical information to parents of non-minor
children without the consent of the patient. In the case of a life
or death emergency, we would contact parents immediately.
Students may sign a release form that gives us permission to
speak to designated people about designated health concerns. In
Indiana, releases are in effect for 60 days.
